  3. Step Three: COMPLETE HEART SCREENING RELEASE & TAPPS PHYSICAL FORMS. BRING ON JULY 19

Parents & Athletes, on Tuesday, July 19 starting at 4:30 PM, we will be having our all-sports physicals on campus at the KCC. The physical will include a heart screening from August Heart and a TAPPS-approved physical by our team physicians. The cost for the physical will be $25 per athlete. All athletes must be registered no later than midnight on Sunday, July 10. 
 
In addition to registering, all athletes must make an appointment with our Athletic Trainer Bobby Ramirez, to record their vitals before the physical date.
 
Athletes are also responsible for bringing completed TAPPS physical forms and physical and heart screening releases.
 
ALL athletes must bring these forms and have them signed by a parent or guardian. We will not be able to print or have extra forms on site.
 
NO FORMS, NO PHYSICAL, NO EXCEPTION!
