It is that time of year again! Flu Season, we will be having a Flu vaccine clinic Oct 12 & 13th.
If you are interested in getting your flu vaccine please fill out the attached form completely and return it to [email protected] or drop off to nurse's office no later than Oct. 10th.
We will only be vaccinating only those who register early and return completed forms. Location will be in the Nurse's Office between the hours of 7:45am-3:15pm.
